Forms / Event Cost

Medical Forms:

All candidates or members participating in an Ordeal or Lodge Event held at a Council camp must have on file with the lodge or submitted with their reservation a SFBAC Annual BSA Health and Medical Record (parts A, B, and C). Parts B & C must be signed and dated. These forms are valid for one (1) year from the date of the Doctor's signature. There are NO exceptions for events held at a Council camps!

A copy of the current BSA Medical Form “Part C” can be found on the Scouting.org Annual Health and Medical forms page.
